• Philadelphia, PA: A 4 month old girl died at an unregistered day care of multiple head injuries.
• Columbia, SC: Two separate incidents at the same day care center of child deaths - both 4 months old - both dead from abuse either by severe shaking or suffocation.
• Gainesville, FL: Three separate incidents at the same facility of toddlers ages 11 months to 13 months - all three with broken arms from the day care provider.
